I recognise the risks of injury that are common to any performing art programs that me or my child participate in and I do hereby waive and release the Association from and against any and all claims, actions, cause of actions, damages, costs, liabilities, expense of judgments, including attorney's fees and court costs, that arise out of my or my child’s participation in this program. I hereby execute this Waiver and Release form permitting myself or my child to participate in the Association's programs. I understand that registration or tuition fees, equipment, testing fees and private lessons are non-refundable. Uniforms, equipment, examination fees, private lessons etc. are all additional costs and are not included in the registration or tuition fees.
